At UltraTrim Clinic, we ask that you view our policies as essential guidelines to maintain the highest standards of service. These policies are in place to provide the best possible experience for all of our valued clients.
Content of the Website​

We kindly ask that you review our policies as necessary standards, not restrictions. Policies such as appointment cancellation fees are essential due to unavoidable operational costs and time allocated to each client. Special offers and promotions can only be used once at a discounted price, and they cannot be combined with other offers or discounts. The promotional services advertised on various platforms are available for one-time purchase at a discounted rate.

Appointments and Confirmations
​
Appointment confirmations are sent 48 hours in advance. It is your responsibility to arrive on time for your scheduled appointment to avoid any cancellation fees.

Reservations
​
Reservations can be made by calling or texting our office during normal business hours. To ensure your preferred time and date, please plan accordingly. A credit card is required to secure your appointment. If you need to cancel, please give at least 24 hours’ notice to avoid a service charge.

Services
​
UltraTrim Clinic offers a variety of health and aesthetic services. Details about each service can be found on our website under FAQs. We require customers to follow all pre- and post-treatment instructions to ensure optimal results. Non-compliance may result in additional treatments or charges, which will be the customer’s responsibility.

Punctuality
​
Please arrive on time for your scheduled appointment. We recommend checking in 5 minutes early. All treatments end at their scheduled time, and unfortunately, late arrivals will not receive extended time. In special cases, and when possible, we may accommodate late arrivals at our discretion.

Payment Options
​
Payment is due at the time of treatment. For packaged services, full payment is due at the first treatment session. We offer various payment options, including credit cards, which will be kept on file for future services and fees. If a treatment series is discontinued, you may receive a pro-rated credit for unused treatments. However, there are no refunds for products or services. The credit may be used for other services or transferred to another individual.

Cancellations
​
To avoid a charge, please cancel or reschedule your appointment at least 24 hours in advance. If no notice is given or if you miss your appointment, a $50 charge will apply for each missed appointment. For multiple or group service reservations, we require a 72-hour notice for cancellations. Prepaid services and gift certificates are also subject to this cancellation policy.

Groupon
​
If you have purchased services via Groupon, please contact Groupon for any inquiries or issues. We will promptly notify Groupon if there are concerns regarding safety or service delivery, and will work to resolve any such issues.

Identity Verification
​
A copy of a government-issued ID will be required for all consultations and treatments. This ensures we adhere to HIPAA regulations and safeguard patient privacy, preventing medical identity theft.

Late Arrival for Appointments
​
Appointments begin and end on time to maintain the schedule. If you arrive more than 5 minutes late, your appointment will be rescheduled and considered a same-day cancellation. In some cases, we may treat you between appointments if time allows, but this is at our discretion.

Return/Refund Policy
​
All products, service packages, and prepaid treatments are non-refundable. If you cannot use a service, you may exchange unused portions for other services, with the original price of the service applied. Gift cards are also non-refundable.
​
Consultations/Treatments
​
Consultations with Dr. John Simmonds are priced at $300, which will be credited towards treatment on the same day. Consultations with other healthcare professionals are complimentary unless a no-show or same-day cancellation occurs. A $50 fee will apply for Good Faith Evaluations (Medical Clearances) required before treatment.

Practice-Patient Relationship

We reserve the right to refuse service at any time, for any reason.

Treatment of Complications
​
While we anticipate positive results, outcomes cannot be guaranteed. In some cases, additional care or treatments may be required, for which additional charges may apply. Please note that your insurance may not cover these additional costs.

Membership Policy
​
Memberships are available but not required to enjoy our services. Memberships are based on a 12-month commitment, and cancellation requires a written 30-day notice. Additional fees may apply upon cancellation. You may receive a full refund if cancellation occurs within 3 days of initial membership purchase.
​
Cell Phones
​
To maintain a peaceful and relaxing environment, please ensure your cell phone is turned off in treatment areas.
​
Promotional/Special Offers
​
All promotional offers expire 12 months after purchase. The amount paid for promotional services never expires, but no refunds or exchanges are allowed. A consultation is required before purchasing promotional services.
​
Emergency
​
We do not provide 24/7 emergency services. Please go to the nearest emergency facility if you require urgent medical care. We recommend maintaining a primary care physician for your regular healthcare needs.

Obtain Medical Records
​
Patients may request their medical records by submitting a release form and a $25 fee. Records will be ready within 72 hours for pickup. Requests must be made in writing to info@ultratrimclinic.com.
​
Financing Program Guidelines
​
Interest is waived if the balance is paid in full by the end of the promotional period. Terms vary based on creditworthiness and loan amount.

COVID-19 Guidelines
​
We are committed to the safety of our clients and staff. We’ve implemented safety measures such as increased sanitization, social distancing, and health screenings to ensure a safe environment. We will follow state and local guidelines as they evolve.
